Registration is required for all attendees of the convention to gain access. You must complete form even if you require no If the attendee is a child under 12 please use the comment box to add names. 

    Member Franchisee Members- 2 free registrations per restaurant
(additional badges $500 each)
   
    Exhibitor / Allied Partner Companies with booth package are entitled to 3 free registrations including ALL meeting meals.  Registration for more than 3 attendees is $ 500 each

    Non-Member Requires $750 registration fee per each person attending

The event will be held at the Bellagio Las Vegas. Contentment and opulence are the hallmarks of your Bellagio luxury hotel experience.

The DFA has exceeded our booked “room block” for the event, and we are now SOLD OUT!

However, to accommodate late franchisee registrations, hotel “over flow” information is available on this confirmation note that will follow after you 'submit'. Please continue to register for the Convention and click continue at the bottom of the page.

*A credit card is required to secure your sleeping room. A hotel room deposit of one night will be charged to your credit card. Cancellations must be received by the hotel no less than 2 days prior to your arrival date to receive a credit back for the deposit. Cancellations & changes to be received in writing via email or fax to DFA office no later than October 17th.

Special Evening Events

Each registered attendee will receive one ticket for each event (based on availability, first come, first serve), as a part of their Badge Registration Package. Tickets must be registered for at this time, and are not secured unless registered for with this form. Please be sure to select the total number of tickets needed below (one per attendee on this form, not to exceed two attendees per form).

Golf Tournament

The DFA Golf Tournament is Sunday, November 2
and will be held at the TPC Canyon Las Vegas Course

8:00am  Tee Time ~ Shot Gun ~ Best Ball

The above items are hosted and sponsored by our golf tournament sponsor, Ventura Foods.  There is a nominal golf registration fee of $75 per player and you will receive more information when you register for the tournament.  You may also reserve rental clubs (at your expense) on your registration form.  Cancellations and changes to be received in writing via email or fax to the DFA office no later than October 17th.  After October 17th, no refunds will be issued of the tournament.

Cancellations & changes to be received in writing via email or fax to DFA office no later than September 28th, 2008. After September 28th, 2008, no refunds will be issued of the $75 tournament fee. As of 9.4.08 the tournament is sold out. You may register to be on a wait list.
