Registration is required for all attendees of the convention to gain access. You must complete form even if you require no hotel room. If the attendee is a child under 12 please use the comment box to add names.

* Room rates are available 3 days before  the DFA Convention start date (Mon, Nov. 8)and 3 days after the DFA Convention end date (Thurs. Nov.11 ) based upon space availability.
* Room rates are subject 12% tax

* Daily Resort Fee of $15 will be added to the Room rates,  which includes: wired and wireless high-speed internet access, Fitness Center access, complimentary in-room bottled water, complimentary individual transportation to designated Grapevine area attractions, Daily newspaper, local and toll free 800 telephone calls

*A credit card is required to secure your sleeping room.

Each reservation will require a non-refundable 2-night prepayment by credit card upon registration and can cancel the reservation up to thirty (30) days prior to arrival to receive a credit back for the deposit. Any cancellations between 29 - 1 day's) prior to arrival will result in a full 100% payment of all reserved nights.

Special Evening Events

Each registered attendee will receive one ticket for each event (based on availability, first come, first serve), as a part of their Badge Registration Package. Tickets must be registered for at this time, and are not secured unless registered for with this form. Please be sure to select the total number of tickets needed below (one per attendee on this form, not to exceed two attendees per form).

Golf Tournament

 

The DFA Golf Tournament is Monday, November 8
and will be held at the Cowboys Golf Club

The above items are hosted and sponsored by our golf tournament sponsor.  There is a nominal discounted golf registration fee of $100 per player and you will receive more information when you register for the tournament.  You may also reserve rental clubs (at your expense) on your registration form.  Cancellations and changes to be received in writing via email or fax to the DFA office no later than October 1st.  After October 1st, no refunds will be issued of the tournament.


 

If you would like to play, a nominal registration fee of $100 will apply for each player participating.

Cancellations & changes to be received in writing via email or fax to DFA office no later than October 1st, 2010. After October 1st, 2010, no refunds will be issued of the $100 tournament fee.
